As an AppleCare member, you have access to a number of important programs that complement the care you receive at your doctor’s office.
Care Management Program
Care Management is an important service to AppleCare members who need a little extra help managing their health care. Your Care Manager will work with you, your family, and your doctors to develop a Care Plan that supports your medical and social needs.
Who should enroll for care management services?
- 1. Members with complex healthcare needs or multiple healthcare issues
- 2. Members who have recently gone to the hospital or emergency room
- 3. Members who would like extra support managing their health care
When You are in the Hospital
AppleCare has put together a team of doctors who specialize in caring for patients in the hospital. Their job is to manage your care throughout your hospital stay as well as coordinate all the after-hospital care you may need, such as home health or durable medical equipment.
When you are living with a chronic or serious illness, your quality of life depends on your quality of care. AppleCare provides Palliative Care to help you find relief from your pain, symptoms, and stress. With our skilled Palliative Care Team by your side, you and your loved ones will receive comfort and peace of mind.
In addition to having great care, it is important that you have support in the other aspects of your life to stay healthy. AppleCare Social Workers can help you access community resources and programs.
As an AppleCare member, you’ll find that getting the care you need and staying healthy is easy. Your AppleCare Primary Care Physician (PCP) will coordinate your health care and makes sure you receive the care you need. Your PCP is “your family doctor” and your first contact if you need medical attention in a non-urgent situation. If you need to see a specialist, your PCP can help refer you to a great doctor in your area.
Make the most of your doctor visit
- Bring all your medication bottles
- Alert your doctor about any allergies or side effects you have experienced, especially from medications
- Write down all your questions, concerns, and previous illnesses and surgeries, including dates.
- Update your doctor about any specialist you are visiting
There is never a convenient time to get sick. That’s why AppleCare Medical Group offers you a large network of Urgent Care Centers that support our patients for after-hours or weekend medical assistance. To find the closest urgent care, click here.
AppleCare believes in keeping you healthy. We encourage you to visit your doctor at least twice a year. If you are a high-risk patient, consult your physician for the guidelines for your specific diagnosis or if a particular illness runs in your family.
Who is AppleCare Medical Group and how is it different from my health plan?
AppleCare Medical Group manages your physicians, both primary care and specialists, processes authorizations, and handles all medical claims.
Your health plan provides your pharmacy and extra benefits, including dental. For a complete list of benefits, please contact Customer Service.
What do I do if I need to see a specialist?
If you feel your condition requires help from someone other than your Primary Care Physician, you need to schedule an appointment with your primary care doctor first, and he or she will refer you to the proper specialist.
How long will my authorization take?
AppleCare believes your primary care doctor knows what is best for you; therefore, authorizations for most routine specialists are automatically approved. For those that require additional review, authorizations can take up to 10 business days.
How will I know when my authorization has been approved?
For authorizations that have been automatically approved, your primary care doctor will give you a copy before you leave their office, and you are free to schedule your appointment immediately. For authorizations that require additional review, a copy will be mailed to your home, so be sure AppleCare always has your correct mailing information.
What do I do if I am not satisfied with my care?
If you feel your care is not to your satisfaction, call our office at 1 (877) 862-1007 and a Customer Service Representative will assist you to make sure you get what is best for you.
